Class Registration Form –
Students should use this form to register for a training course.

Novice Training Log –
Students should use this form to record their training hours during novice trainings. Course instructors should record evaluation notes as appropriate on the form at each training.

Training Report Form –

Designated Trainers use this form to report on their training sessions and for student registration at the class. This form is used for all classes or trainings that will be sent electronically to the JHPTS Administrative Assistant for inclusion in the database. NOTE: Confirm all information is correct. Data from this form will be used by JHPTS to create the pre-populated student Evaluation Sheets after classes or trainings that require evaluations.

Training Report Form Directions

The directions are provided to help guide you in filling out the form correctly. Please read and follow the directions as you fill out the form. This will make the job of transfering the form data to the database much faster and easier.

Course Evaluation Sheet – Certified Trackers

Course Instructors use this for local evaluation of previously certified students. NOTE: In most classes, this form will be created by the JHPTS Administrative Assistant and pre-populated with student information from the Class Pre-Planning and Registration Form that was sent to JHPTS. The form is available here for trainers that need a blank copy for special uses.

Advanced Tracker Evaluation Form

Use this form for evaluating students attending a special Advanced Tracking class. NOTE: In most classes, this form will be created by the JHPTS Administrative Assistant and pre-populated with student information from the Class Pre-Planning and Registration Form that was sent to JHPTS.
